After Sim Update 7, 787 Aircraft performance with minimum fuel, and weight. The Aircraft doesn’t maintain speed or altitude even with full engine trust.
Temp Spikes of 264 C degrees.
Not sure if you have AntiIce on, but you could be experiencing airframe/wing icing which does degrade the performance.
I recently did a flight from CYEG-PHNL with 90% payload without any problems using a step profile flight plan. It was snowing and blowing when I left CYEG and I had to have anti-ice on cause the airframe was getting iced up on the ground.
Btw, I was using the default version and not the “heavy” mod
The Anti-Icing was on, so, it should not have any problem.

I don’t know what your gross weight was. For high gross weights FL390 is above operational ceiling.
