Oh, I see what you’re saying now. Yeah, I think I may have heard ATC give me a QNH setting above the transition altitude, now that I think about, despite me flying at BARO STD.

Yes, if you keep the STD BARO above 5000ft in Germany for example, it will not match the altitude ATC is asking you to be. Below 18,000ft you should use the QNH to avoid having the ATC reminding you to be at the correct altitude. This is not how it works IRL, but until having the ATC improved taking in account those different transition altitudes, this is the way how you should proceed.
