Screen Tearing changes based on game focus. NVIDIA RTX4090

Greetings,
This situation began today after an MSFS crash during boot. The sim reset back to defaults and ran the setup screens as if this were a first time install.
The new problem:
I am experiencing that when the I have focus set to the display running MSFS that I have very noticeable screen tearing when scrolling around the cockpit. When I move the mouse to change focus to a secondary monitor the screen tearing in the cockpit stops. There was no screen tearing prior to the reset event regardless of focus. Can someone provide a technical explanation why this occurs and if there may be a resolution?

I had previously made screenshots of my in sim “GENERAL OPTIONS > GRAPHICS” and used them to change the GRAPHICS values to those I was using yesterday.
MSFS Display: Samsung 75" 4K TV running at its 60Hz refresh rate.
Secondary Display for simconnect apps, etc.: MSI MAG281URF gaming monitor running at 144Hz.
GPU: NVIDIA RTX4090
CPU: 13900KS
RAM: 64Gb DDR5

This is not new. From time to time game window is not properly detected which makes v-sync or g-sync stop working and tearing is clearly noticeable. Switching to windowed and back to full screen may solve the issue but normally you need a complete game restart, so that detection is reset.

When using 2 or more monitors you may try to force v-sync globally at Nvidia control panel. That will still keep v-sync active (forced) both at windows desktop and game when you switch focus from game to second monitor and viceversa, even if game focus detection fails.
