I think a large part of the problem is that we live in such a highly litigated society now that it pretty much prevents this. The primary goal is always to limit liability so things like honesty and actually solving problems is secondary to that. Its the reason it takes a year of paper work to fix the pot hole on the street in front of your house.
I will often jokingly say “it cant be my fault I don’t do anything” when there are problems at work. Sometimes people get the joke but often not. At any rate it is where we are at.
