ATC is nagging me to "descend to 12,000ft, Mission Steps showing reach and descend…
Altimeter is standard, since above 10,000ft.
(but to be precise, if I set the altimeter to what ATC is giving me, 30.46, all is good).
So the bug apparently is that the standard altimeter is not set above FL100.
I do also wondered here. Now I just don’t use standard barometer any more.
If you are told to go something feet, it’s always not standard. If you are told to go flight level (FL), then you set baro to standard.

While the last is correct, FL is standard, ft is Baro, over 10,000ft it should be standard.
Or is it 18,000ft in US and 10,000ft in EU (or else)?
Depends on where you are in the world. The USA uses 18,000 feet as it’s transition altitude. In Europe and much of the rest of the world it’s much lower.
From what I’ve seen the game uses 18,000 Ft everywhere.
OK, then it’s wrong to use 18,000 in EU, because here we usually have 10,000ft as transition alt.
