Did you set the interrupt priority of the graphic card to high? would be via MSI mod utility though…
Having promised myself I would go back to MSFS 2020 for a while I noticed a few comments about turning off Fauna. I gave it a go and suddenly all the stutters and audio mess was gone. Relieved in a way (and thankful) but also frustrated that one little setting could cause such havoc. (and it was not like I had turned it on - it has always been on)

I have the same problems . i have a 5090 and cpu is intel i9-14900K .in the beginning the first 5min of flying in VR are really good and smooth but after few minutes it stutters badly .the fps run down to 18fps and sound stutters too . if i look around sometimes the stutters are gone and the fps are really good again but if i look in other corner the stutters come fast back . its not playable . i remember i had the same problem with the 4090 too. I must say when MSFS2024 was launched it was perfekt in VR with the 4090 gpu but now i only can play in non vr mode . Is it the game or is it the driver . i think the game needs updates
Have you tried turning off ‘Fauna’. It has worked for a lot of us
What has worked for me is turning off ReBar in Nvidia Inspector for the Microsoft Flight Simulator 2024 profile. For 2020 it defaults as off, for 2024 it defaults as on. I run HAGS off too so I don’t know if that also affects that combination.
