Start a flight plan in midair

I would like to start a flight plan in mid-air, either over water or over land, without taking off from an airport. Is that possible either using World map or Little Nav Map?

You won’t be able to load a flight plan once you have clicked the “FLY” button. You can, however manually enter a flight plan into your nav at any time during flight and activate it.
If you want to fly IFR you then just need to request clearance from ATC.

I believe you spawn wherever you wish, by just clicking on the main map. I don’t think you even need to set a destination. If you want to be flying on a certain heading, one trick is to set your destination in the direction you wish to face. You can set the altitude you spawn at in the NAV log on the same screen.
