Swiss Cheese Model of Accident Causation Applied to PNG's Worst-Ever Air Crash

In this video we look at the Swiss Cheese Model of Accident Causation and apply it to the worst-ever air crash in Papua New Guinea - the crash of Airlines PNG Dash 8 flight from Lae/Nadzab to Madang in October 2011. Hope you find it interesting. Cheers.

Thanks for this. Pilots are typically interested in / follow news of incidents, accidents and crashes. You learn from them and apply the lessons so as not to repeat them.

Since starting to use MSFS a little over a year ago, I’ve found that recreating such scenarios – both the famous ones (such as the crash you’ve depicted here) and general “no no’s” that have been the cause of numerous mishaps throughout aviation history – make for some interesting sim sessions.
