Do you have any piloting assists turned on?
They could be in either or both the main Assistance Options, or from the in sim toolbar.
If not, if you have anything in your community folder, try with it empty.
If you have done the above, and if either happens at random times, go to the Controls Option page,
Click on search by input, wait and see if anything is displayed.
If so, you are getting a command from that particular button or key unknown to you.
That can be the result of wear and tear, as well as contamination. etc.
What controller(s) do you use?
I run a Logitech X56 HOTAS and were experiencing very similar issues, not just in MSFS, but also in IL-2. After some online research I found it’s a known issue if both the joystick and the throttle are plugged into the same USB controller (mostly a nearby port). So I just moved one of them from the neighbor USB 2.0 port to an USB 3.0 port (because I was too lazy to research the USB ports-to-controllers map). This resolved it completely for me and I hope it helps!

I have the same X56 setup and will try your recommendation.
Have you mapped your X56 configurations to MSFS as yet? I have just done a simple configuration for my throttles FLAPS GEAR and SPEED BRAKE.

Thanks for you suggestions. Tony also suggested installing controllers into separate USB ports (I have the same controllers as he - X56.
Will try this next. The only Pilot Assist I have on is the Tower communication only.

PROBLEM LOOKS LIKE IT IS NOW FIXED
Having the controllers installed to separate USB ports works. I had the same controllers as you X56 but had them installed on a USB hub.

Great news and I’m glad that it worked for you! On the X56 mapping I found the default MSFS layout that loaded automatically not that bad, but I have tweaked it a lot (just in the sim) bit by bit since then. Happy flying!
