The reverse thrust thing is already a thing. You can map a button to hold to engage reverse thrust or a button to press to engage reverse thrust.
The axis for left engines and right engines is something I can get behind I suppose. But like you said, you could just make a profile for 4 engined planes and switch to that profile whenever you fly them. Only takes a few seconds to switch from profile to profile in msfs and you could consider it part of your flight prep.
What I do is have my left throttle represent engine 1 and 3, and my right throttle represent engine 2 and 4. I rarely use differential thrust. So that tends to be my primary throttle distribution setup. Especially since it preserves the logic that engine 1 should be the left throttle and engine 2 should be the right when flying in two engine aircraft (which you’ll find yourself in far more frequently than four engined aircraft).
That being said, I’ll vote for it anyway as a L and R engine axis would be somewhat useful albeit not critical IMO.

That is the right setup option, but as yet you haven’t assigned a button to any of the reverse thrust options. Personally, I set up my throttle so one of the buttons activates the bottom option there.
You could map two buttons to “toggle throttle reverse thrust”. That would mimic the effect of having an on and off button.
It would be nice if we can control each engine separately.
It will be perfect if we can assign an axis to the same throttle/thrust lever or to another axis with another lever. In this way, we can control the amount of reverse power, like idle revers.
e.g.
The reverse isn’t On/Off.
