I had that happen once somewhere, KAVX (Catalina Island), if I recall correctly. The plane taxied right off the edge of the runway and face-planted into the drainage area near the runway. However, once I clicked Ready to Fly, I spawned into the cockpit on the runway as expected.
It seems let the animation run long enough, the AI can get confused and dumb things happen.
