Just started up the engines for the first time, and after a couple minutes I get red warning saying engine malfunction and the engine shuts down, with N2 and N3 still running though. If I try again it malfunctions about 30 seconds in. Both engines.
What could cause this?
What was the outside temperature ?
There is a known issue if temperature is below 6c, startup with some aircraft will fail.
hard to tell from the screenshot but looks like exactly -6c as this was an airport in Norway. What is the procedure for cold startups with this issue? Would turning off all damage help?
Unfortunately I am not aware of a procedure, maybe some users are. The only workaround is to setup a custom weather with an outside temperature > 6C or to start at an airport which Live weather report a temperature >6C.
You should report it to Inibuild support since A350 is a 3rd party aircraft.

From my testing, it seems to be the fog that causes “overstress damage” to be applied to the engines. Disabling engine stress damage in the options maybe helps. What definitively helps is to enter Dev Mode and pause or disable the wear and tear system (you can see a screenshot in my A320 bug report)
The issue (for the A350) has already been reported to iniBuilds, but I think there’s not much they can do about it because in the end it’s an Asobo system that applies damage to the engines.

Just curious, when you are having fog that damages the engines, could the temperature be above 6C or is it always below 6C ?.
I mean I don’t think it’s the cold weather alone that causes the damage. When I did troubleshooting for the ini A350, I tried using the 24-hour weather history and changing the time and during nighttime when it was still cold, there was no overstress damage. It was only when the fog kicked in during the morning that I started seeing damage. This was a month ago so I don’t have exact details anymore.
Trying to create similar conditions in the weather editor did not help to reproduce the issue. It seems like it only happens with live (or 24 hour historic) weather.
Regarding if it happens with temperature above 6°C, I can’t say anything.

I assume the fog is the reduced visibility reported in the METAR since there is not really a fog simulation (as per my understanding). Using custom weather there is no setting regarding visibility but I remember having done some tests using custom weather with low temperature (no fog) and was able to reproduce the issue with the A320 & FA-18.
OP here it was indeed very foggy as well as cold (norway morning) during the engine failing incident
